Who Is Pair Team?

A California medical group that gives people with Medi-Cal a free, personal care team.

What Pair Team Is

Pair Team is a medical group based in San Francisco, founded in 2019 by a registered nurse and an engineer. It is an Enhanced Care Management (ECM) provider for Medi-Cal health plans under California's CalAIM program. Pair Team's mission is to transform how care shows up for people who have historically been failed by the system.

How Pair Team Delivers Care

Care teams are led by community health workers hired from the communities they serve, working with nurses, nurse practitioners, and behavioral health care managers. Members get help by phone, text, video, or in person - at home, at clinics, or in shelters - in English or Spanish. Pair Team partners with local clinics, shelters, food pantries, and community organizations like THCC.
